the ECFC staff has requested that you download and fill out forms, then this is the correct page.
Please make sure you have all forms printed. Read through all of
them and have parental signatures for MINORS.
All documentation must be filled out in ink, signed and notorized.

You will recieve an email or phone call confiming your entry.

Please make sure you mail these forms no later than 2 weeks prior to the event so that we receive them in a timely manner.
If there are any problems, please contact us. The forms will need to be completey filled out
and notorized with parental consent before you can participate.
